Southern Surgical Arts offers a secure login to our electronic medical record so that you may complete your new patient forms in the convenience and privacy of your home.  The Online Patient Portal saves you time before you appointment, and is a safe method of keeping your information current at our office.

Just click on the computer icon below, and  you will be directed to a new window where you will be asked for your security code.  Please call our office and we will email the security code to you.  Then, you will be able to create your own username and password.

Pre-Operation Forms

The Pre-Operation Form is solely for cosmetic surgery patients of Carey Nease, MD, at Southern Surgical Arts before surgery at either the AAAHC accredited surgery center in Calhoun or a local hospital. It is not intended to be medical advice for any other persons and should never take the place of advice from your personal physician.  This information is given at the pre-op appointment for patients of Dr. Nease at Southern Surgical Arts. If you have any questions or concerns, please call our office to speak with one of the nursing staff.  If you have a medical emergency, please call 911 now or go to your local emergency room.
